- details of any grant funding that your club or community group might apply for in 2026/27
- usage information – the days and months the leased premises is used, and for how many hours per week
- information about any other groups who use or sub-let premises
- details of any capital works or maintenance undertaken in the last financial year, or planned in future years (350 words max.)
- Public Liability Insurance Certificate of Currency; and
- current evacuation plan for the leased premises.

Why is the City of South Perth requesting information?
The City requires all community leaseholders to provide documentation as per the requirements of their lease.
Is the provision of information mandatory?
Yes. All relevant information requested in the compliance form must be provided, with the form submitted by Friday 17 April 2026.
Who should complete the compliance form?
An authorised representative from your club, group or organisation should fill out the compliance form. This could be the President, Secretary, Treasurer or another office bearer or committee member.
What information will I need to complete the compliance form?

How long will it take me to complete the compliance form?
Once you’ve gathered all the relevant information and electronic copies of the documents, the compliance form should take approximately 15-20 minutes to complete.

Can I save my progress and continue later if I exit the survey?
