The City requires all community leaseholders to complete an annual compliance form. As well as meeting the terms of the lease, it is a great opportunity for us to get to know you and your group better and to improve the services we offer.

The information you provide will help us:

  • Learn more about your club or community group
  • Improve our delivery of development services for your club or community group
  • Share training and workshop opportunities to build capacity and help your group grow
  • Assist you with grant applications and funding opportunities
  • Assess the use and condition of the City’s lease facilities
  • Ensure building compliance throughout the City of South Perth.

An authorised representative from your club, group or organisation should fill out the form, for example the President, Secretary or Treasurer.

You will need information to hand on contact details, membership numbers, ABN and/or Incorporation Number, building usage, financial statements, public liability certificate and more to complete the form. It’s helpful to gather all the information you need before you begin – read the full list in the frequently asked questions (FAQs).

Once you’ve gathered all the relevant information and electronic copies of the documents required, completing the form should take approximately 15-20 minutes.
