Why is the City of South Perth doing this audit?

    The City is asking all community leaseholders to complete this audit form. This is a great opportunity for us to get to know you and your group better and to improve the services we offer.

    The audit will help us:

    • Learn more about your club or community group
    • Improve our delivery of development services for your club or community group
    • Share training and workshop opportunities to build capacity and help your group grow
    • Assist you with grant applications and funding opportunities
    • Assess the use and condition of the City’s lease facilities
    • Provide support to ensure building compliance throughout the City of South Perth.

    When will the audit take place?

    The audit will take place on Your Say South Perth from Thursday 22 May to Thursday 19 June 2025.

    Who should complete the audit form?

    An authorised representative from your club, group or organisation should fill out the audit form. This could be the President, Secretary, Treasurer or another office bearer or committee member.

    What information will I need to complete the audit form?

    Before you begin, it’s helpful to gather the following information which you’ll need:

    • the address of the leased premises
    • the best contact details to use - email address, postal address, phone number
    • membership numbers for the current and last financial years
    • a brief description of the benefits your club or community group provides (350 words max.)
    • your club, community group or organisation’s ABN or Incorporation Number
    • details of any affiliations your club or community group has with State or other governing bodies
    • whether your club or community group has a reserve sinking fund
    • details of any grant funding that your club or community group might apply for in 2025/26
    • usage information – the days and months the leased premises is used, and for how many hours per week
    • information about any other groups who use or sub-let premises
    • details of any capital works or maintenance undertaken in the last financial year, or planned in future years (350 words max.)

    You also need to have electronic versions ready to upload if your club, group or organisation has any of the following documents:

    • constitution
    • most recent Annual General Meeting (AGM) Minutes
    • Public Liability Insurance Certificate of Currency
    • audited financial statements for last financial year
    • Long Term Financial Plan (LTFP)
    • Strategic Plan or other forward planning document
    • First aid equipment compliance certificate
    • current evacuation plan for the leased premises.

    It’s best to use PDF or Word file types, and please keep your file sizes to 5MB or less. You might have to rename files which have long names or special characters to a simpler name before you can upload them. You can only upload one file per question.

    How long will it take me to complete the audit form?

    Once you’ve gathered all the relevant information and electronic copies of the documents, the audit form should take approximately 12-15 minutes to complete. It may take longer if you have a lot of electronic documents to upload.
